Failure Mode and Effects Analysis (FMEA)

FMEA is a step-by-step approach for identifying all possible failures in a product and then prioritizing them according to their potential severity and likelihood of occurrence. In order to identify potential failure modes, engineers must first understand how the product is intended to function. They then identify all the ways in which the product could fail to meet its intended purpose. Once all potential failure modes have been identified, they are evaluated according to two criteria: the severity of the consequences if the failure were to occur, and the likelihood of the failure occurring. The product is then redesigned to eliminate or mitigate the most severe and likely failure modes.
